1. Services Covered

I consent to receive diagnostic testing and related services (“Genics Physician Review Services”) provided by Genics Laboratories, LLC (“Genics,” “we,” “our,” or “us”) and its contracted independent physician group, Nevada Health Services LLC, located at:

These services may include:

• Physician review and approval of my test request(s);
  
  
• Collection, performance, and analysis of my specimen(s);
  
  
• Review and release of my results (“Results”);
  
  
• Optional telehealth consultations or educational sessions (“Consults”); and
  
  
• Care coordination, counseling, or education where permitted by law.
  
  

All medical decision-making, including approval of test orders and review of Results, is provided solely by Nevada Health Services LLC and Dr. Anurag Singh, M.D., as an independent physician contractor.
The physician is not an employee of Genics Laboratories, and this agreement does not create a partnership or joint venture.

2. Acknowledgements and Agreements

I understand and agree that:

• I have reviewed and understand the information provided about the test(s) I am ordering.
  
  
• The information I provide is true, accurate, and complete.
  
  
• I authorize Genics Laboratories and Nevada Health Services LLC to share my health information for the purpose of providing the Genics Physician Review Services.
  
  
• Genics Physician Review Services do not replace care by my personal physician.
  
  
• If a result is abnormal or requires prompt follow-up, Nevada Health Services LLC or the Genics Care Team may contact me by phone, secure message, or mail.
  
  
• I am responsible for sharing my results with my own physician and arranging any necessary follow-up.
  
  
• Certain results (e.g., positive sexually transmitted infections or other reportable diseases) must be reported to public-health authorities as required by law.
  
  
• I will not delay seeking medical advice based solely on my test results.
  
  

Payment / Physician Service Fee

I understand that each test order requires a physician review and authorization provided by Nevada Health Services LLC / Dr. Singh.
Genics Laboratories charges a flat $5 physician service fee for this review, which covers medical oversight and result authorization.
This fee is non-refundable once my order has been reviewed by the physician.

4. Telehealth Consent

If I participate in a telehealth Consult:

• Nevada Health Services LLC will determine whether telehealth is appropriate for me.
  
  
• Consults may occur via phone or video; they will not be recorded.
  
  
• I may withdraw my consent to telehealth at any time by contacting  [email protected]
  
  
• Technology-based encounters carry risks, including interruptions or data breaches.
  
  
• I may need in-person evaluation or treatment.
  
  

5. Alternative Options

I understand I may obtain testing through my own physician, urgent-care clinic, or other medical facility.
I voluntarily choose Genics Laboratories’ DTC testing.

6. Communications

I authorize Genics Laboratories and Nevada Health Services LLC to contact me via email, text, phone, or mail regarding my tests, Results, and follow-up.
I will notify Genics Laboratories of any changes to my contact information.

7. Privacy

My health information will be handled according to the Genics Laboratories Notice of Privacy Practices, available upon request. Genics Laboratories complies with HIPAA and applicable state privacy laws.

8. Limitation of Liability

To the maximum extent allowed by law, neither Genics Laboratories nor Nevada Health Services LLC / Dr. Anurag Singh shall be liable for any indirect, incidental, special, or consequential damages arising out of or related to the Genics Physician Review Services.
Total liability shall not exceed one hundred dollars ($100).

10. Consent

I have read this Informed Consent carefully, all my questions have been answered, and I voluntarily agree to proceed.

INFORMED CONSENT TO PERFORM HIV TESTING

(Applies only if you order an HIV test or STD panel that includes HIV testing.)

I have been provided information and understand that:

• HIV attacks the immune system and, if untreated, can cause AIDS.
  
  
• The HIV test detects antibodies and/or viral material in my blood sample.
  
  
• Testing too soon after exposure may yield a false-negative result (“window period”).
  
  
• If I believe I was exposed within the last 72 hours, I should immediately contact a physician to discuss post-exposure prophylaxis (PEP).
  
  
• If my test is reactive, Nevada Health Services LLC will contact me to discuss results and treatment options.
  
  
• All HIV testing performed by Genics Laboratories is confidential but not anonymous and requires identifying information.
  
  
• Positive results are reported to the appropriate state health department as required by law.
  
  
• Discrimination based on HIV status is prohibited by law.
  
  

I hereby consent to be tested for HIV.

TERMS OF USE & NOTICE OF PRIVACY PRACTICES

A. Terms of Use

• Non-Emergency Use: Genics Services are not for medical emergencies. Call 911 for emergencies.
  
  
• Eligibility: You must be 18 or older and in the U.S. when using the Services.
  
  
• Payment: DTC testing is self-pay only. No insurance, Medicare, or Medicaid billing.
  Each test order requires a $5 physician review fee, covering oversight by Nevada Health Services LLC / Dr. Singh.
  
  
• Physician Oversight: Nevada Health Services LLC reviews and approves test orders, evaluates Results, and may provide telehealth Consults.
  
  
• Consult Scope: Limited to reviewing Results, education, and limited treatment where allowed by state law.
  
  
• Disclaimers: Services are provided “as is” without warranties.
  
  
• Indemnification: You agree to hold harmless Genics Laboratories and Nevada Health Services LLC from any claims arising from misuse of Services or Results.
  
  
• Governing Law: Nevada law governs these Terms.

B. Notice of Privacy Practices (HIPAA Summary)

Your Protected Health Information (“PHI”) may be used or disclosed for:

• Treatment — communicating with physicians and laboratories.

• Public Health Reporting — as required by law.
  
  
• Legal Requirements — subpoenas, court orders, etc.

Your Rights:
You have the right to access, correct, or copy your PHI; request restrictions; receive an accounting of disclosures; and request a paper copy of this Notice.
