Thyroid Comprehensive Panel

Test Overview

The Thyroid Comprehensive Panel is a detailed set of blood tests designed to evaluate thyroid function and overall hormonal balance. This panel includes measurements of thyroid-stimulating hormone (TSH), free and total thyroid hormones (T3 and T4), and thyroid antibodies to assess for thyroid dysfunction, autoimmune conditions, and metabolic health. It provides a more in-depth analysis than a standard thyroid panel, helping detect conditions like hypothyroidism, hyperthyroidism, Hashimoto’s thyroiditis, and Graves’ disease.

Test Types

  • TSH (Thyroid-Stimulating Hormone) – Regulates thyroid hormone production.
  • Free T4 (Thyroxine, unbound) – Active form of T4 that influences metabolism.
  • Total T4 (Thyroxine, bound and unbound) – Measures total thyroxine levels.
  • Free T3 (Triiodothyronine, unbound) – Active form of T3 that regulates energy and metabolism.
  • Total T3 (Triiodothyronine, bound and unbound) – Measures total T3 levels.
  • Reverse T3 (rT3) – Helps assess thyroid hormone conversion and stress-related dysfunction.
  • T3 Uptake – Evaluates thyroid hormone binding in the blood.
  • Thyroid Peroxidase Antibodies (TPOAb) – Indicates autoimmune thyroid disease (Hashimoto’s or Graves’).
  • Thyroglobulin Antibodies (TgAb) – Helps diagnose autoimmune thyroid conditions.
  • Thyroglobulin (Tg) – Monitors thyroid cancer or inflammation.

Expected Turnaround Time

  • Results Available: 1–3 business days

Specimen Requirements

  • Sample Type: Blood sample (venipuncture)
  • Container: Serum separator tube (SST)

Collection Instructions

  1. Fasting is not required, but some doctors may recommend it for consistency.
  2. Avoid biotin supplements for at least 24–48 hours before testing, as they can interfere with results.
  3. Inform your healthcare provider about medications, especially thyroid hormones, steroids, and birth control pills, as they can affect hormone levels.
  • Storage: Blood samples should be processed promptly and stored according to lab protocols, typically refrigerated or at room temperature until testing.

Test Details

Clinical Use: Diagnoses thyroid disorders: Helps detect hypothyroidism, hyperthyroidism, and autoimmune thyroid diseases. Monitors thyroid treatment: Ensures proper dosing of levothyroxine (Synthroid) or anti-thyroid medications. Assesses autoimmune thyroid diseases: Identifies conditions like Hashimoto’s thyroiditis and Graves’ disease. Evaluates thyroid cancer or inflammation: Monitors thyroglobulin levels after thyroid cancer treatment.

Limitations: Thyroid levels can fluctuate due to stress, illness, and medication use. Autoimmune thyroid markers alone do not confirm a diagnosis and should be interpreted alongside hormone levels. Pregnancy and menopause can impact thyroid function, requiring additional monitoring.

The Thyroid Comprehensive Panel provides a detailed analysis of thyroid health, helping with diagnosis, treatment monitoring, and long-term endocrine balance.
